    Over 20% of GP appointments are for non-medical issues such as housing, finances, lonliness and emotional support. 

    Our GP websites and Social Prescribing hub-sites enable GP practices to utilise their websites as powerful channels for supporting patients to self-manage their health and wellbeing.

    We provide a dynamic and affordable web template with an emphasis on supporting people to self-manage and providing navigation and signposting to social prescriptions.

    The approach has been co-produced with GPs and is based around local Social Prescribing Hubs, which hold resources, support and service info on a central database.

    This makes local information easy to maintain and enables it to be automatically updated on any GP website that use the KYOH GP website template.
