We bring to the world of self-management a unique combination
As well as experience in health coaching to support self-management and patient activation, the founders of Know Your Own Health bring experience of creating online resources for the NHS and Department of Health, having worked on a variety of projects from 1996 to 2010. These included the award-winning ‘D-Code’, a drugs education behaviour-change programme, as well as providing online support for the Expert Patient Programme and other behaviour change programmes.
In 2010 Know Your Own Health was set up to support self-management initiatives, providing online tools and collecting outcomes data using an online structure to support pain management programmes from 2012 and health coaching programmes from 2013. This combination has enabled data-collection in terms of the inputs (health coaching as an intervention) and outcomes (patient activation measurements, and other quantitative and qualitative outcomes) for patients. This has enabled more in-depth learning and experience to be gained around how to effectively deliver supported self-management interventions to achieve the desired outcomes.
