A specially trained health coach works with the individual (or group) on the non-clinical aspects of their health that may be stopping them from living their life or managing their health in the way they would like.
The focus is on what's important to the individual in managing:
People typically have five or six sessions with a health coach. Although the coaching is usually through face-to-face sessions at times to suit people's individual needs, there are a range of other health coaching options people can choose from so they can structure the coaching in the way that works best for them, for instance: telephone, skype, online or group.
Having completed the coaching they can then remain part of the service and continue to access information, self-management tools and access any additional support that's available to them.
