There is substantial international evidence to show people who are supported to self-manage effectively have better outcomes, a greater sense of wellbeing and make more effective use of clinical services. This means healthier, happier patients and lower clinical costs.
The approach we use is personalised and completely non-prescriptive. Patients decide for themselves what they want to do and how they want to do it. For this reason, people are more motivated to make the changes they need to make. The support enables them to take the necessary steps to get where they want to be.

Who is it for?
The coaching benefits individuals or populations with any health condition that can be positively impacted through better self-management and/or lifestyle changes.
All service users are also given access to an online patient platform. Our services - coaching and/or online platform - can also be adapted to work with a specific long-term condition.
